Persons certified by medical professionals as being unable to walk without assistance or who have restrictions caused by lung disease, defective vision, or cardiac, arthritic, neurological, or orthopedic conditions are eligible for handicap parking placards.
Applicants/guardians must complete an Application for Disability Parking Placard (MR-37A) and have a physician complete the relevant section of the application certifying the applicant’s disability. Applicants are required to furnish their proof of identity for obtaining a placard.
Both permanent and temporary placards have a $5.00 fee.
Permanent placards are valid for 5 years. Temporary placards are valid for 1 to 6 months.
Permanent placard holders must receive recertification from a physician before a new placard can be issued at expiration. Temporary placards cannot be renewed.
Lost, stolen, or damaged placards can be replaced for a fee of $5.00. Simply visit the local DMV and request a replacement there.
The placard must be hung from the rearview mirror or placed on the dashboard on the driver’s side if there is no rear view mirror.
Each eligible applicant is issued two placards.
